Title: Dipper...Here's a POA

Dipper we use the General POA in our house...and contrary to what SOC said all of our 3rd parties use it.  School, church, psychologist, Dr.s, dentist, police have (on occasion) taken it.  So it might work for you.

Hi Sunshine!  Thank you....I think we will use the limited power of attorney.....but, there isnt a place for the person giving rights to sign on that one, I guess I will add it to be on the safe side.  I may be overlooking it as well...

I had held on to the letter for the counselor explaining that dh only wants our household to be responsible for scheduling appointments....after we get this form notarized, i will just mail it in with the letter....
