Soc,

Can you tell me what form to file for a change of venue in CA. I have looked and cant find the form number.

There's no specific form #. You would just use FL301, check the "Other" box and specify "Change of Venue." Then you check "Other" in 3.(f) and attach a declaration in support of the change, stating the facts that the court should consider in granting your motion.
