I am going to send a modified version of the "request for medical records" from this site.

1. Should I, or should I not, include that the person who took my daughter to this visit had no legal right to fill out any of the paperwork involved in this medical visit, nor had any legal right to discuss my daughter's medical history (to the effect that this physician is in breach of confidentiality?)

Well...I guess since I offered to continue taking the children to appointments and was refused(cuz it isn't my visiting time)...then Dad should be the one taking them to their appointments, not step mom.

So I guess it's just a payback thang??? Shame on me.
