I would think she has to at least meet you at the old exchange location.  If it goes back to court, I would ask the judge to order her to pay your gasoline expenses since she decided to move farther away.

First does you boyfriend have any rights (court set)?  Second if he doesn't he needs to file a complaint for visitation.  Since you have the address that is the address you send the paperwork to, she'll probably respond and if not then you file another document called Requet for Order of Default,  that should get you started.
