Child Support Review Proccess - HT proceed / child safety in danger

I am a single mother - my little girl is almost 7 months old.  Biological father has not demonstrated sincere desire to assist with support or be a consistent/dependable part of this child's life.   

I have received one email and one voice mail from biological father since he was notified of the conception (conception occurred in Sept 2008; he was notified in Oct. 2008)

After researching his recent history, I discovered at least 2 DUI convictions, driving without a license, and sexual assault on multiple females (all appear passed out in pictures). 

I am currently receiving Tanf and food stamps after layoff in April 2009. 

Critical Issues: 

- have received notice by the Attorney General that I am required to attend a Child Support Review Process within the next two weeks.

- have desire to terminate biological fathers parental rights, or require supervised visitation and random UA / Drug testing on a monthly basis.

- believe my daughter would be in danger if left unsupervised with this person.
