DH has court ordered visits during spring break, holidays, and summer.

SD is due to give birth (she is 16) in April, therefore the spring break visitation is basicly out....  

1) What about the summer??  Can we NOT have SD and the baby for the visit for the summer per the court order??

A court order is not optional. You are entitled to exercise custody per the court order. Period. Now, if SD is in labor, then the other parent or guardian can't be in contempt because they are not acting voluntarily in preventing your visitation, but otherwise, pregnant or not, you are entitled to visitation, absent further orders of the court.
