Dear Soc,

      I had a question earlier this week on proper format to motion court for mediation.

     I am in Iowa, I am pro-se and ex is represented by attorney.

     Just wanted to have you look these over, and let me know if I am correct in this.

In support of the above stated motion, I, JOHN DOE Respondent, after first being duly sworn, do hereby depose and say as follows:

1.  The issues currently before the Court are:

     A.  Petition to Modify Custody and Visitation.

     B.  Make up time for denied visitation.

     C.  Petitioner not following Court Order

     D.  Petitioner allowing minor child to be in a vehicle with an unlicensed driver.

     E.  Denials of phone calls and home phone number.




2.  I believe that I, the Respondent, JOHN DOE, am not so estranged from the Petitioner, JANE DOE, nor Petitioner from myself, the Respondent, that we cannot resolve the above stated issues via Mediation.

3.  For all the foregoing reasons I hereby request that the Court order Petitioner and myself, the Respondent, to engage in Mediation within thirty days (30) from the date of this entry of the Court's Order on this Motion.  Mediator to be chosen by mutual agreement of Respondent and Petitioner, one half of the Mediation costs to be paid by each party, said Mediation to be completed within sixty days (60) from the date of entry of this order.

Reviewed and edited. In general, good. No reason to sign and date a document twice when you're swearing to it in the presence of a notary.
