I'm filing a petition of modification custody and child support as an "per se"
I'm looking for examples of what to put in the motion. I move from AZ to NC to be close to my 2-1/2 yrs old little girl. This petition will be filed in Guilford County, NC.

Go to your local court house and pick one up (fill-in),or if you sit there and wait they will help you fill it out (not legal advice but fill-in what you say). You should give examples of why you custody and child support should change. Child support should be where the child lived for the past 6 months and it is by guidelines for that state. If the amount is not correct from your last years taxes (or recent paystubs) you can ask for a modification.
