Will this be garnished for back child support arrears?  Does anyone know??  Will it require a form to be filled out or will our 2007 tax forms tell the government all they need to know and who qualifies for what?

They basically made a handshake agreement. They haven't even agreed on how much extra per child a taxpayer will get.

If it is like the 2001 tax rebate checks, they will garnish for arrears. Like I said, it isn't exactly finalized but that seems like a reasonable assumption.

For the 2001 rebate no extra forms were required. They just used the info on the 2000 tax returns filed. I'm not sure what they will do about whether they will go by the 2007 or 2006 tax returns as a rule. With tax extensions, it will push some tax payers until after the date they project to get the check out (June) to even file for 2007.
