visitation reduction

Hey, just need some info. please. I had my hands on a form to help calculate a visitation reduction & have not seen it in maybe a year. I downloaded and printed it free off of a website and now, I have no idea where. Can some one please help me find one? I want to show dh's ex what we could ask for in a reduction and see if we can't come to a compromise.......  Thanks
