question about asking for more visitation

So how would I go about filing for more visitation with my son? Our divorce was done in UT, she now lives in AZ and I live in NC. I have him every other Christmas and Thanksgiving and 30 days out of the summer. I want to file to get him ALL summer, spring break, all chrismas break and 3 weekends out of the year in months I don't have him....
How do I file for this?

how old is he?  you should also be alternating spring vacation from school and probably have him all over the except for two weeks (so mom can have a vacation too).
